Medicare Notice of NonCoverage

Important Message from Medicare

The following healthcare facilities are required to give advance notice to Medicare beneficiaries and to members of Medicare Advantage plans before the care is terminated, and to inform them of their right to request an appeal:

• Nursing home
• Comprehensive outpatient rehabilitation facility
• Home health
• Hospice
• Hospital (inpatient status only; the appeals process does not extend to care provided while under observation)
