Extra Support Available for PSPC Participants

The nationwide Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) helps integrate evidence-based clinical pharmacy services into the care and management of high-risk, high-cost, complex patients.

Now in its fourth cycle, the highly successful PSPC has opened to a broader group of participants—enrollment is no longer limited just to HRSA-funded organizations. 

 

Take advantage of Qualis Health's assistance

Under our Medicare Quality Improvement Organization (QIO) contract, Qualis Health is currently providing technical assistance to PSPC 4.0 participants and gearing up to assist PSPC 5.0 participant teams in Idaho and Washington. Download the related flyer.

To be eligible for our free technical assistance, teams must identify a cohort of at least 50 Medicare beneficiaries, for whom you provide ongoing care, who meet at least one of the following criteria:

  • Take the anticoagulant warfarin on a regular basis
  • Take short-acting or long-acting antipsychotics
  • Take hypoglycemic medications for diabetes mellitus

Teams are welcome to focus on a single population, or on multiple populations, based on the priorities of your organization.

Our assistance is available free of charge, and is in addition to the substantial resources offered through the national PSPC structure.

PSPC 4.0 Change Package
Outlines the best-practice interventions that participants in the 2011-2012 Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) are using to reduce adverse drug events among high-risk patients. The change package is focused on five over-arching strategies: Leadership Commitment, Measurable Improvement, Integrated Care Delivery, Safe Medication Use Systems, and Patient-Centered Care.

Provides an overview of the successes achieved during the 2010-2011 Patient Safety and Clinical Pharmacy Services Collaborative.
