Improving Cardiac Population Health through Primary Care Redesign

High-performing primary care and cardiology practices across Idaho and Washington are invited to join Qualis Health’s Cardiac Learning & Action Network (LAN): "Improving Cardiac Population Health through Primary Care Redesign."

The Cardiac LAN is unique:

Takes advantage of remote learning and online collaboration—which is less labor-intensive than traditional Collaboratives.

Provides an opportunity for the region’s most technologically advanced practices to establish quality benchmarks—you will be setting the bar for other practices to strive toward.

Creates reporting efficiencies—the same measures that participants report to the Cardiac LAN can be used as submissions for Medicare’s PQRS incentive program.

Employs expert staff with solid experience across all three domains of health IT, payment reform, and clinical quality improvement—our real-world curriculum can be put to work right away.

Uses Medicare funding to cover faculty costs and online events—Qualis Health’s services are provided at no cost to you.

Expectations

This initiative is designed to get results. It requires the commitment and desire to make real progress.

As part of this initiative, the Qualis Health team will:

  • Provide individualized consulting to improve your team’s ability to address cardiac population health.
  • Deliver educational opportunities via a combination of in-person and remote webinars, workshops, and other events.
  • Encourage peer-to-peer networking among similar participating practices.
  • Assist practices in workflow redesign and quality improvement methodologies to improve outcomes for cardiac care.
  • Provide benchmark reports that allow practices to track progress toward goals.
  • Guide practices in building clinical and PQRS reports from the EHR.
     

Participating practices will be expected to:

  • Submit data on 6 PQRS measures monthly to Qualis Health.
  • Submit 3 PQRS measures to CMS for incentives.
  • Designate a team to participate in onsite and remote learning activities, including an in-person, statewide kick-off event.
  • Provide resources to support the team— including time to attend learning sessions, implement changes, and participate in phone calls.
  • Share best practices and lessons learned.
