Primary Care Practices
As part of our Medicare Quality Improvement Organization contract, Qualis Health helps primary care practices:
1. Use electronic health record (EHR) systems to drive improvement in cardiac population health. Practices often feel stretched to provide resources for multiple improvement projects. Our new initiative, "Improving Cardiac Population Health through Primary Care Redesign," is designed to build on existing efforts while meeting a single important clinical goal. Let us help you find synergies across multiple priorities and make substantial improvements.
2. Improve health outcomes and reduce ADEs among high-risk populations, by providing additional technical support to participants in the national Patient Safety and Clinical Pharmacy Services Collaborative (PSPC 4.0).
3. Improve the safety of care transitions, such as designing workflows to ensure patients recently discharged from a hospital receive timely follow-up care from their primary providers. (Read about opportunities to participate in these community-based projects.)
4. Respond to Medicare beneficiary complaints.
