Pressure Ulcer Learning and Action Network

Your efforts to prevent pressure ulcers just got more support!

Qualis Health is working with Idaho and Washington nursing homes in a  Learning and Action Network focused on preventing pressure ulcers in high-risk, long-stay residents.

The Network is designed to harness the power of collaboration, spread practical implementation of best practices, and connect participants to national initiative activities and resources—all with an aim of achieving bold improvements in nursing home quality of care.

Participating nursing homes are committed to:

  • Participating in each of the learning sessions and action periods
  • Assessing current practices, testing improvement ideas, implementing effective changes, and using data to track progress
  • Ensuring that improvements become embedded into everyday practice
  • Using an interdisciplinary, collaborative approach and apply evidence-based practice
  • Engaging in peer-to-peer problem-solving of common challenges to implementation
  • Building on current quality assurance program by learning and implementing systematic performance improvement approaches

Registration is now closed. (See sidebar for contact information.) The Network kicks off in November 2011 and will be actively engaged in quality improvement efforts through December 2012, at which time participants will monitor their MDS Quality Measures for sustainability through October 2013.

Goals of the Learning and Action Network:

Commit to eliminating all unnecessary pressure ulcers.

Collaborate with peers to decrease pressure ulcer rates by 20% or more.

Assess current quality improvement program and identify and select opportunities for improvement.

Prepare for new Medicare Conditions of Participation requirements for improved Quality Assurance/Process Improvement (QAPI) programs.
