Qualis Health is a proud supporter of The Partnership for Patients: Better Care, Lower Costs—a public-private campaign aimed at improving healthcare quality and reducing costs nationwide.

Currently, one in seven Medicare patients is harmed during the course of care—which translates not only into a tremendous human cost, but also a financial one. It is estimated that the healthcare-associated harm costs approximately $4.4 billion each year.

Funding for care transitions projects

As part of the Partnership initiative, CMS is committing millions of dollars nationwide to communities focused on helping patients safely transition between settings of care. Funding was made available through the Community-based Care Transitions Program (CCTP).
