The current session of the Workflow Strategies for Using Your EMR to Improve Care training series offers the following continuing education opportunities to participants of the live teleconference. Participants must complete a related evaluation to receive the credit.

List at least one medical home standard used by NCQA for recognition of practices as patient-centered medical homes
Describe at least two ways in which medical home concepts can impact delivery of preventive services and other kinds of population-based healthcare
Identify at least one change idea related to effective delivery of preventive services and/or other population-based healthcare which you can implement in your practice
