The Agency for Healthcare Research and Quality (AHRQ) Patient Safety Survey

As a critical part of the Patient Safety project, CMS asks providers to survey their staff twice during the three-year period using the AHRQ Patient Safety Survey. This survey, which has already been extensively researched and utilized in hospitals, has been modified slightly to accommodate the needs of nursing homes as well.
All providers participating in our patient/resident safety work will be expected to administer and score the survey twice over the three-year period.
Deadline for Baseline Survey
If your facility has recently conducted the AHRQ Patient Safety Survey, please notify Paula Parsons. You may be able to use these data as the baseline sample.
If you have not recently conducted this survey, please prepare to administer and score the survey as soon as possible between now and December 2008.
Training and SupportQualis Health will provide support in these activities, including a WebEx training session regarding how to administer and score the surveys.
