Patient Safety Culture Survey

Please note: The AHRQ surveys have recently been renamed as HSOPSC (Hospital Survey on Patient Safety Culture) and NHSOPSC (Nursing Home Survey on Patient Safety Culture).

All nursing homes and hospitals participating in our Medicare-sponsored patient safety initiatives will be expected to administer and score the survey twice over the three-year project. It measures several aspects of patient safety within a facility.

Survey Data Entry & Analysis Reporting
Hospitals 
If you are administering the survey yourself, you may find it helpful to use a predefined Microsoft Excel tool to enter your data and automatically graph results. The analysis tool is provided free upon request.


Nursing Homes
If needed, please contact Paula Parsons for assistance in scoring your survey.

 

National Comparison Database

Hospitals
Hospitals who have completed the AHRQ survey can compare their results with those from across the country. View the 2010 report, which includes responses from 885 hospitals.

In addition, hospitals can use an Excel template to graph their organizations' results against the nationwide 2008 scores.

Nursing Homes
A similar database is also in the works for nursing homes; details will be available soon. Preliminary results from the 2008 pilot study have been released.

 

Action Planning

Hospitals
Hospitals who have completed the AHRQ survey use this planning worksheet (Microsoft Word document) to create an action plan.
